When the vocalist proclaims that he is “stuck underwater”, that’s another way of saying that he’s caught up in some type of depressed or stressed out state. And basically, what the lyrics center on is his desire for someone to come and help alleviate him of this situation.
That someone, as implied by the first verse, may be an individual he’s had a loving relationship with in times past, a person who was “always around to make (him) smile”. But apparently there is no one in the vocalist’s life like that at the moment, thus this song.
