To kick someone when they’re down is an idiom which basically means doing something detrimental to a person while they’re already in a troubled position. And in this case, it is the singer himself who is in such a predicament. Moreover the person kicking him would be a female, most expressly it seems a romantic interest. And comprehensively, what he appears to be arguing is that ladies can be a troublesome lot. Or stated differently, he perceives women as the types mentioned above – ever ready to add insult to injury. And in acknowledgment of this reality, the primary sentiment of the song has him inquiring why such is so.
