Others have mentioned that it had to do with his step father being very sick and eventually dying and how he felt about that experience. However, during an interview with Rolling Stone, he confirmed the song was about appreciating an ex.
Based on the lyrics of the song, his fans believe the track is about Kendall Jenner.

I think personally this song is a mixture of feelings about an ex and wanting them back as well as a drug problem at the time. Who knows maybe the ex didnt like him doing the morphine. One of the lines in the song he says specifically “give me the morphine, is there any more to do”- something like that. So I think he was struggling with the drug problem and his ex not wanting to be around him, which could have stemmed from the morphine use. Just a thought.
