 “Blood, sweat and tears” is actually a popular idiom which points to the idea of hard, painful work going into an endeavor before making it successful. Or more generally, it alludes to the notion of sufferation. And apparently, the romance which is at the center of this song is one which has its fair share of challenges. But making that reality known to the listener is just the set up for the sentiment Ava Max really wants to relay.
